Assumption of Responsibility, Risks and Liability Waiver
I understand that there are certain risks of damages and injuries, including and not limited to, death, inherent in the practice and play of flag-football, and I am voluntarily willing to assume these risks. These risks include but are not limited to those hazards associated with weather conditions, travel, playing conditions, equipment, all COVID-19 related incidents, and other participants.
By filling this waiver, I understand that the nature of playing flag-football is hazardous and risky, including but not limited to the acts of running, jumping, stretching, sliding, diving, and collisions with other players and with stationary objects, all of which can cause serious injury or death to me and other players.
Liability Waiver & Indemnification
In consideration of approval to participate in this sport, myself and any of my personal representatives, heirs and assignees, hereby waive any rights relating to and release, and forever discharge Elite Flag-Football League and the Permit holder for the same, its committee members, members, volunteers, staff from any and all liability, including without limitation, actions, cause of actions, negligence, claims and demands for damages or loss or injury resulting from or arising out of my participation in basketball. I also agree to indemnify and shall hold harmless members of Elite Flag-Football League from any and all actions, causes of actions, demands, expenses or losses whatsoever which may arise from my negligence or willful malfeasance by my participation in any event, which results in damage to my and all property and any all personal injuries, including death of others or myself.
Physical Altercations & Illegal Activity on Permit Property
Elite Flag-Football League is passionate about bringing people together to engage in physical activity and to pursue a healthy lifestyle in a fun and effective way. Elite Flag-Football League has a strong ZERO TOLERANCE policy when it comes to physical altercations on permit property. Any player that physically engages another player or players can be subject to a minimum suspension of 1 game. Depending on the seriousness of the conduct in question, the player or players may also be banned from the league for the rest of the 2023-2024 EFFL seasons. There will be no money refunded to a player who is suspended for however many games or banned from the league. Elite Flag-Football League is not held responsible for any damages or incidents related to the law that the player may cause on their own.

By marking "I Agree" I confirm that I will not physically engage another player or players. I confirm that I will not engage in any illegal activity on permit property. I understand if such an event occurs, Elite Flag-Football League has every right to suspend me from a minimum of 1 game to being banned for the rest of the 2023-2024 season, I understand no money will be refunded to me what so ever if such events occur. I understand Elite Flag-Football League is not held responsible for any damages or incidents related to the law that the player may cause on their own.
Media Permission Form
EFFL will be taking photographs and videos of the players and posting it online on our website and our social media platforms such as Instagram, Snapchat, TikTok, and Facebook. This form gives us your consent to post these pieces of media online for marketing purposes.
By marking "I Agree" I, confirm I am above the age of 18 and I give full permission to MCBC and its staff to take photographs and videos of myself which will be posted online to the website, and the social media platforms such as Instagram, Snapchat, and Facebook.
Refund Policy/Non-Payments
We want to ensure transparency and clarity regarding our refund and non-payment policies. Please review the following policy carefully:
Refunds:

Refunds will only be issued in the event that the football season does not take place. In such cases, a refund will be provided to all registered teams/free agents.
Non-Payment: 2. All participating teams/free agents are required to pay the balance due by the specified due date on the league's registration page. Failure to make the payment by the due date will be considered non-payment.
Consequences of Non-Payment:
3. In the event of non-payment, the following consequences will apply:
a. The non-refundable deposit submitted by the team during registration will be retained by the EFL.
b. The team will not be able to participate in the upcoming football season.
c. The team will be ineligible to compete in any games or events (including but not limited to tournaments)organized by the EFL until the outstanding balance is settled.
d. If a team/player has not paid their balance by the due date and the season is set to begin in less than 7 days, the league will retain the team/player deposit and said team/player will not be eligible to participate in any games or events even if the balance has been settled.
Communication and Reminders:
4. We understand that unforeseen circumstances may arise, which could affect a team's ability to make the payment by the due date. In such cases, we encourage teams to communicate with the EFL as soon as possible to discuss potential solutions or alternate arrangements.
5. The EFL may send reminders or notifications regarding payment deadlines to registered teams via email, phone, or any other contact information provided during registration. However, it is the responsibility of the team to ensure they are aware of the payment deadline and fulfill their financial obligations accordingly.
Dispute Resolution:
6. In the event of any disputes or questions regarding payments, refunds, or non-payment, the EFL will work in good faith to resolve the matter in an equitable manner. We encourage open and honest communication to find mutually agreeable solutions.
Amendments to the Policy:
7. The EFL reserves the right to modify or amend this refund/non-payment policy at any time. Any changes made will be communicated to all registered teams in a timely manner.
By participating in the football season organized by the EFL, teams agree to abide by this refund/non-payment policy. It is essential to read and understand the policy before registering and making any payments.
